Be Our Guest

For good and valuable consideration, I hereby grant LIFTinnovate LLC (“LIFT”) and its subsidiaries, affiliates, agents, designees, successors and assigns, the irrevocable and absolute right and permission to use my image, photograph, and/or any other likeness or indicia identifying me (my “Likeness”) as embodied in one or more recordings, videos, photographs, and/or other images, in connection with LIFT’s marketing, production and media activities in any and all media, including via the internet or on broadcast advertising. I understand that my Likeness may be edited, copied, exhibited, published or distributed and waive the right to inspect of approve the finished product wherein my Likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording.

I agree that I will not hold LIFT, or anyone who receives permission from it, responsible for any claim or liability, past, present or future, resulting from the use of my Likeness in accordance with the terms hereof, including, but not limited to, claims in the nature of defamation, disparagement, slander, false light invasion of privacy or publicity or the like, and I hereby release, hold harmless, and forever discharge LIFT from all claims, demands, and causes of action, past, present or future, which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have, have had, or may have in connection with LIFT’s use of my Likeness as set forth herein.

By signing this form, I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby.
